Visit Types
Initial – New Patient Consultation $320
60 min duration
During this consultation, Dr. Broszko will perform a complete medical and psychiatric assessment. Dr. Broszko will spend time with you to discuss your mental health needs and devise a treatment plan to work towards these goals.
Dr. Broszko will discuss his findings and suggest treatment options. This is an opportunity for the patient and Dr. Broszko to decide if the treatment plan outlined is satisfactory and establish a working relationship.
Any medications prescribed will be automatically sent to your pharmacy.
In addition, Dr. Broszko may request additional collateral information from your previous medical providers, family or significant other, to help formulate a diagnosis and treatment plan.
This would only be done with your direct written consent.
Follow up – Established Patient $120
20 min duration
During this evaluation Dr. Broszko will review and revise your mental health needs. Above all, if there are any changes that need to be made Dr. Broszko will discuss these with you in detail and the rationale behind his treatment recommendations.
Administrative Requests/Letters/Forms/FMLA/DMV
$110 / per hour
There may arise a time that specific documentation/paperwork is requested to be completed. Please discuss forms to be completed with Dr. Broszko during your appointment. Forms can be submitted through your patient portal to be completed electronically.

How to schedule a visit.
1. Select your Visit Type (New Patient Visit or Follow-up Visit)
2. A calendar will pop-up revealing all available appointment slots.
3. Select a time and date from the calendar.
You will receive a confirmation email with your scheduled appointment time.
If you are a New Patient, you will also receive an Invitation to Register your CHARM – Connect Care Psychiatry Patient PORTAL.
